EXERCISING YOUR CONSUMER RIGHTS TO KNOW OR ACCESS, DELETE OR CORRECT

- The Right to Correct: You may contact Statara to correct any incorrect personal information

If you are requesting to know or access, delete, or correct information and are making that request on behalf of a minor or another individual as an authorized agent, you cannot use this form. By exercising your right to delete, you will limit or cease the collection and use of personal information for targeted advertising by third parties. This may include some or all forms of behavioral advertising, including, but not limited to, public service announcements, political advertisements, or any other advertisements using targeted groups.
To delete or correct information submitted, Statara MUST be able to confirm the identity of the individual making the request. The more information you provide, the greater the likelihood of making a data match in our systems. Statara cannot act on any request to know, access, delete, or correct personal information without confirming the requestor’s identity.
